Men’s Wellness Membership Agreement
Program Overview
Welcome to the Hush Men’s Wellness Membership — a program designed to support your health, vitality, and overall well-being with personalized medical care in a refined, discreet setting.
Membership Investment
Initiation Fee: $400
- Covers a comprehensive lab panel, medical consultation, and full interpretation of results by your provider.
Monthly Fee: $150
- Covers ongoing medical visits, drawing of basic lab work, medication management (typically compounded testosterone replacement therapy), and exclusive membership benefits.
Membership Benefits
- Comprehensive medical visits related to men’s wellness and hormone health
- Drawing of basic laboratory work and full interpretation by your provider
- Ongoing medication management, including basic medications such as compounded testosterone
- 10% discount on skincare products at Hush Medical Spa
- A complimentary annual Aquafirm facial for you or your spouse (or an item of equal or lesser value)
Membership Terms
- Membership is month-to-month and may be canceled at any time with written notice before the next billing cycle.
- If no medical treatment is recommended following initial lab evaluation and consultation, the member may opt to cancel the monthly membership payments without further obligation.
- The $400 initiation fee is non-refundable and covers the cost of initial laboratory testing and medical evaluation, regardless of treatment outcome.
- Members must maintain an active credit or debit card on file for monthly billing.
- Unused services do not roll over month-to-month.

Important Notes
- Health Savings Account (HSA) or Flexible Spending Account (FSA) funds may be eligible for reimbursement for medical visits and labs. Please consult your benefits provider for confirmation.
- If you have completed recent appropriate lab work within the past 60 days, Hush Medical Spa may review those labs and adjust the initiation fee at its discretion.
- The complimentary Aquafirm facial may be gifted to a spouse or applied toward an item or service of equal or lesser value.
Acknowledgment
By enrolling in the Hush Men’s Wellness Membership, I acknowledge that I have read and understood the terms above and agree to the initiation and monthly recurring payment structure as outlined. I understand that I may cancel monthly membership payments if no medical treatment is recommended after initial evaluation.
